Jeesey Peasey - that worked! Desktop now appears on BOTH HDMI and LCD.

I had seen that Viewtopic before but passed on it because a) the LCD in the post (the red one) is not the Waveshare SKU18366 (2.4inch) but a physically larger LCD that is one of the two dozen or so that I have been experimenting with but is too big for the case we will be using and b) it was early in my search and I clearly did not realise it's significance at the time.

LCD needs a little tweak or two - top taskbar is missing for instance but I think I can get the active taskbar on the LCD with an arandr settings - no I'm not seeing any option there. Or it could be the width-mm and height-mm settings as the 2.4inch SKU18366 LCD has an active screen area of only 47mm wide by 27mm high compared with the config setting of 57x43 I will see if a config.txt edit will show the toolbar on the LCD although from memory I cannot remember seeing an active toolbar on BOTH screens at the same time, I do however have vague memory of being able to move the active taskbar from hdmi to LCD in a one of the fbtft driver instances.

Big thank you to @Bugsworstnighmare for making me look again! And it absolutely meets the requirement of no kernel kung fu ! Just plain vanilla 64 bit Bookworm, an easy to install dtbo, and a simple edit of config.txt. Mission accomplished. Now to see if with some tweaks it will work with the 2.8inch Waveshare SKU27579 which uses the st7789v controller. This SKU has the added complication of a Capacitive Touch screen which we will tackle as a separate issue - I'm just interested in getting the LCD dispaly working first.
